As Past Chief of Plastic Surgery at the Trillium Health Center, Dr. Lista enjoys privileges at Mississauga’s primary hospital, a mere 3-minute drive from The Plastic Surgery Clinic. If you prefer to have your operation performed at the hospital, we’re more than happy to accommodate your request.
No matter which location you choose, Dr. Lista will personally perform your procedure. Your anesthetic will be administered by a highly trained, fully certified anesthesiologist who stays with you throughout the surgical procedure and remains with you as you wake up to closely monitor your progress. If you have any questions or concerns in the hours and days following your operation, a surgeon will always be on call, 24 hours a day.
The Breast Enlargement Operation
The operation takes between one and two hours to complete. Only a small incision is necessary to insert either saline or silicone breast implants. Before surgery and during your free consultation, Dr. Lista will discuss your incision placement options. Depending on your body, your lifestyle, and your type of implant, there are advantages and disadvantages to each placement. The incision, about an inch in size, can be inconspicuously placed at the bottom of the outer edge of the nipple (the lower edge of the areola), under the crease of the breast itself, hidden in the armpit, and in some cases, hidden in the bellybutton. Dr. Lista will be able to advise you on which approach will be best for you, and in the end, you get the final word on the location of the incision.
Breast Augmentation Sitting Up
Dr. Lista understands that his patients lead busy lives—they’re not women who live life lying down. Because of this, his breast augmentation patients are prepared in an upright sitting position. This allows him to perform the operation and evaluate the position and the look the breast implants will have for the patient in real life. Since our goal is to always deliver the most natural looking result possible, it’s important to optimize that outcome for the way the breasts are most likely to be seen.
For your comfort, you are put put to sleep lying down on our state of the art operating table. Once you are safely under anesthesia, our nurses gently secure you into position and maneuver the table into its upright sitting position. Dr. Lista will frequently step back and examine the results, comparing them to pre-operative photos and making sure the finished result will match the goals for size and shape you discuss before surgery. This process can take a little more time and a little more work than the traditional approach to breast augmentation, but we think it’s worth it to deliver exactly what you want.
Immediately Following Breast Augmentation Surgery
It’s important to make arrangements with a friend or relative to come pick you up after your operation. You will also need to have someone stay with you for the first 24 hours after your surgery to help with your earliest stage of recovery. For your health and safety it’s important that you not be alone. You will also have two small drainage tubes attached to your incision sites to help prevent fluid build up, reduce swelling and ensure the lowest risk of complication possible. When you come to see us the following day, the drains will be removed in a process that’s quick and painless.
You will also have the benefit of the Pain Relief Pump™ right after your surgery. This device is about the size of smartphone and you will leave with it attached to a belt. Through nearly microscopically thin tubes, it delivers a long-lasting local anesthetic to your surgical site that prevents you from feeling any pain. This can help you to relax and feel much more comfortable after surgery when it’s so important for you to rest easily so that your body can start to work on healing itself. Since you don’t feel the pain, one of the biggest benefits is that you more than likely won’t need to take prescription pain medications. Sometimes these types of pills can have side effects like nausea that lead to vomiting, and some patients even have allergic reactions to them. All of these side effects impede the healing process and put unnecessary strain on the body. If we can avoid that, we can help you have a faster and happier recovery with less pain and swelling.

Types of Breast Implants
There are two main types of breast implants to choose between: saline and silicone. There are a number of factors that determine which type is right for you, your lifestyle, your goals, and your body. Dr. Lista will help you understand which implant will give you the most natural look and feel with the lowest risk for complication.
Saline Breast Implants
Implants filled with saline are a good fit with certain patients because they require the smallest incision possible. The implants arrive at our clinic from the manufacturer already empty and that’s how they’re inserted into the breast. Once they’re been placed, they’re filled with saline solution, a naturally occurring fluid in the human body. This fluid is the same solution given to hospital patients by IV drip to help with rehydration. In the very rare event that the implant ruptures, there is no danger to the patient and the saline is absorbed and naturally processed by her body.
Cohesive Gel Silicone Implants
These are the newest generation of silicone breast implants. For a period of time several years ago, many women were hesitant to opt for silicone implants due to complications and health risks that had arisen with the earliest versions of these types of implants. Thankfully technological advances allowed for major developments, leading us to where we are today. Plastic surgeons all over the world have been using cohesive gel silicone breast implants for years with great results and totally safe outcomes. The silicone inside the implant is in a solid gel state instead of a liquid. This means that if the implant shell ruptures, the silicone remains in its solid form and does not leak. This type of implant can be a great option for very thin patients who don’t have a lot of breast tissue to begin with. Problems such as rippling and the ability to feel the implant can occur with saline implants, but silicone implants eliminate these issues. However, this implant type also requires a slightly larger incision since it can’t be inserted empty, which can contribute to a more noticeable scar.

Position of the Breast Implant: Over or Under Muscle?
When the breast implant is placed in the breast, it can either be positioned over the pectoralis muscle or under it. So how do we figure out which position will work best for you? As with all breast implant choices, it depends on a few interrelated factors.
Benefits of Under the Muscle Placement
Positioning the implant underneath the muscle can create a soft, natural breast, the main benefit being that the muscle helps cover the edges of the implant making it less visible. It’s a particularly good technique for patients who are thin and who have no droopiness to the breast. It can help to decrease the ability to feel the implant and is less likely to result in rippling. For very athletic women—body builders and fitness models, specifically—this placement may not be ideal. When the pectoral muscles flex with high intensity, as with a muscle building weight training workout, the implant can be affected and move as a result.
Benefits of Over the Muscle Placement
This placement means that the breast implant sits right behind the breast tissue and is not covered by muscle. Women who have more breast tissue to begin with can get a really soft and natural result with this technique since that extra tissue helps to adequately cover the implant. Patients whose breasts have some droopiness or sagginess, the medical term for which is ptosis, are also better suited to have their breast implants placed above the muscle. However, there can be downsides to this approach, as there is more of a risk of rippling, hardening, and external visibility with this placement.

Your Office Visit & Consultation
Consultations with Dr. Lista are always provided free of charge. He wants every patient to have access to as much information as possible before making a decision that could affect their health, well-being, and appearance. Sometimes a non-refundable consultation fee can put unnecessary financial pressure on the decision-making process when prospective patients should be able to relax and comfortably make the decision that is truly right for their body and their lifestyle. In consultation as well as in surgery, his prerogative is to always do what’s right for the patient.
Because of this, it’s Dr. Lista’s preference to see each of his patients for at least two visits before their breast augmentation operation. There are a number of things that need to be accomplished during the consultation phase. A lot of time is spent performing a comprehensive physical examination and obtaining a through medical history to ensure that you’re a good candidate for this procedure. We also want to give you an opportunity to ask him all of the questions you have about the operation, no matter how big or small they might seem. Some patients find that other questions come up after their first visit, and so a second consultation gives you the opportunity to address every concern and feel confident and comfortable in your decision, whether you decide to proceed with surgery or not.
This is also an important chance to discuss what you hope to achieve with breast augmentation. By discussing your particular goals and expectations, Dr. Lista can work with you to totally customize your surgery to achieve a beautiful, natural looking breast that matches the size and shape you want. At the same time, it’s essential that you go into your procedure with realistic expectations about your potential result. Not all expectations can be met and that’s why we want you to have a careful discussion beforehand. If Dr. Lista can’t meet the expectations you have, he won’t perform the procedure: it’s essential that patients are not set up for disappointment.

Choosing the Right Breast Implant Size & Shape
There are a number of ways we can help you determine which breast implant size will be right for you. Bringing in photos of the look you like can help us to visualize your goals, and our specialty computerized digital imaging allows Dr. Lista to show you different examples of how you might look after surgery depending on which implant volume you choose. We offer our patients different sizes of bras to try on to get a better idea of their goals. When you hear an implant measurement like 325cc, let’s say, to describe the size and shape of a breast implant, it’s difficult to visualize what that looks and feels like in the real world. That’s why we also give our patients sizers to test out with a sports bra so they can understand the look and feel of size variations.
We want you to feel comfortable to spend as much time as you need with Dr. Lista to discuss your size expectations. He will offer you a range of sizes to choose from that will give you the enhancement you’re looking for, but will still ensure that you’ll achieve a result that looks natural. Because, while choosing the right size is important, it’s not the most important thing.
The most important factor in breast augmentation is to give you a natural looking breast. It should look and feel natural, it should suit your body, and it should look like you were born with that shape. There is no single implant size, shape, or type that will create the best result for every woman. But there is an implant somewhere out there with the ideal combination of all of those factors that can create the best result for you. Breast Augmentation Recovery
Before your breast augmentation surgery, you will need to make arrangements with a family member or friend who can drive you home after your operation. You will also need someone to stay with you for 24 hours following your surgery for your health and safety.
Recovering after breast augmentation is an important part of your procedure. By making sure you get as much rest as possible, you will help your body through the healing process. For this reason, we typically recommend that you take one week off of work. That way you can focus on rest and relaxation where you spend your time at home in bed reading, watching TV, or catching up on Netflix. During this time, driving is not advised. It’s really important that during your initial healing period you’re not having to run around after small children. If you do have young ones in the house, be sure to arrange for childcare during this week.
When you’re a busy woman who’s accustomed to an active schedule, it can be difficult to feel like you’re dropping everything and doing nothing. But taking on too much too soon can actually prolong your healing process and make you feel like a less effective person for a longer period of time. Committing to a quiet, calm early recovery period can help you achieve the best results possible from your breast augmentation and can even speed up your overall recovery. It’s really worth it.
You can return to gentle exercise about two weeks after surgery which includes walking and low impact cardio activities like the stairmaster. You can return to running, high impact exercise, and upper body workouts after about one month.
Dr. Lista will discuss all the ins and outs of recovery with you during your consultation. You’ll be given materials that will help guide you so you won’t have to worry about forgetting anything. The most important thing is to follow his guidelines carefully to ensure a safer, easier breast augmentation recovery.
